Safe evidence record supporting an adapter compatibility decision.

The record is immutable and thread-safe. Evidence is designed for audit and review reports, not for replaying operations. Metadata is redacted and must not expose raw messages, artifacts, commands, credentials, private keys, or endpoint secrets.
